Core20PLUS5 is a national NHS England approach to inform action to reduce healthcare inequalities at both national and system level. The approach defines a target population – the ‘Core20PLUS’ – and identifies ‘5’ focus clinical areas requiring accelerated improvement.
 

The approach, which initially focussed on healthcare inequalities experienced by adults, has now been adapted to apply to children and young people.

Eastmoor Community Project and St Mary’s Community Centre in partnership with Live Well Wakefield deliver a CORE20PLUS5 Social Prescribing project to adult residents of Wakefield district.

 

The project supports local residents to improve their health and wellbeing and reduce health inequalities across the Wakefield District.

Many adults are impacted by social barriers and non-medical needs that impact their ability to take up and attend health care appointments, and adhere to health care plans, with leads to poor health outcomes.

 

A team of our volunteers are going to support clients to attend their local health care appointments overcoming social barriers such as independent travel skills, health literacy and social anxiety.

The team hopefully will not only support clients to attend their appointments but also improve the clients’ overall health and wellbeing, and reducing social barriers.

 

Also we have weekly drop in sessions at St Swithun's Community Centre every Thursday between 11am -1pm to find out more about the project. Ask for Julia

Referral Criteria

Referrals can be made on the Live Well Wakefield website or through calling the Live Well Wakefield office and the community centres

 

  •          18+

  •          History of missed appointments or likely to miss appointment.

  •          Referrals must be residents of Wakefield District

  •          Must be able to finance their own travel and self-care.

 

What appointments?

 

  •          Any hospital appointments at either Pontefract Hospital or Pinderfields Hospital

  •          GP appointments

  •          Any other healthcare related appointments within your area (dental, ears, eyes etc)

 

Information to gather for the referral

 

  •          Name, DOB, address, Phone Number

  •          GP practice registered to

  •          Appointment details: Time, Date, Location

  •          Reason for needing support.

  •          Preferred method of contact: Phone, Letter, Text Message, Email
